Practicing in post-acute and long-term care (PALTC) is a distinct specialty—even within geriatrics—requiring both clinical expertise in the care of older adults and a strong understanding of care systems, insurance structures, and regulatory requirements. Clinicians in PALTC settings must also apply subspecialty knowledge in complex, real-world contexts, as residents often face limitations in mobility, cognition, and tolerance for interventions that make routine outpatient specialty care challenging.

This course will explore the unique clinical and operational demands of PALTC practice, emphasizing the integrated specialty and subspecialty knowledge needed to deliver high-quality, age-friendly care.

Objectives
At the conclusion of this course, learners should be able to:

  1. Identify key biopsychosocial factors contributing to the complexity of care for older adults in long-term care (LTC) and analyze at least two common barriers to accessing or delivering specialty medical care in this setting.
  2. Outline how to integrate sub-specialty knowledge and practices to complex older adults accounting for multimorbidity and geriatrics syndromes such as falls, polypharmacy, and frailty
  3. Review strategies for collaboration amongst interdisciplinary teams in coordinating care to meet the specialized medical and social needs of older adults living in LTC, clearly defining team roles and communication strategies.
