Overview

Format: Self-paced online

An epidemic of opioid misuse, abuse, and overdose is occurring in Minnesota and across the country. In Minnesota, hospitalizations related to opioid use are increasing, there has been a four-fold increase in the number of opioid-related deaths over the past decade, and admissions for treatment of substance use disorders are on the rise. A critical need exists for healthcare professionals to be more aware and educated about best practices and guidelines to effectively manage acute, subacute, and chronic pain and the use of opioids across the pain continuum. The purpose of this continuing education activity is to educate prescribers who manage all phases of pain on the best practices in opioid prescribing and pain management.

The course will include practical tips and tools for providers to use to have more effective conversations with patients regarding pain management. Strategies to effectively manage acute and subacute pain will be reviewed with the goal of alleviating pain and reducing the chronic use of opioids. Best practices in the management of patients on chronic opioid analgesic treatment will be presented. The course includes content on non-pharmacological and implantable device alternatives for the treatment of pain, and a review of the Minnesota Opioid Prescribing Guidelines released in 2018.

Course Structure

This course consists of an introduction, four modules with assessments, and a final assessment. The steps to complete the course and receive credit are:

  1. Register for the course by clicking on the "register".

  3. View each module and complete the associated attestation or assessment. Modules must be completed in order.
  4. Complete the final assessment.
  5. Complete the course evaluation and access your Certificate of Participation through My CE in your CE Portal account. Your Certificate of Participation will also be emailed to you.

The passing threshold for assessments throughout the course is 70%. Multiple attempts are permitted.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Apply current Minnesota Opioid Prescribing Guidelines and best practices across the pain continuum (acute, post-acute, and chronic) to effectively manage pain and reduce opioid use disorder and dependency.
  2. Implement interviewing techniques to facilitate difficult conversations with patients regarding their pain management.
  3. Identify, implement and educate patients about alternative approaches to treat pain including multi-modal non-pharmacological and implantable device alternatives.
  4. Describe and implement a pain-management plan for patients on chronic opioid analgesic therapy.

Module 1

Approach to Difficult Conversations with Patients About Opioids and Pain Management

 Dr. Kaz Nelson

This module will discuss best practices in having a patient-centered, productive, efficient and high yield conversations related to management of pain. It is important to realize that patients presenting with pain have activation of the amygdala (‘fight or flight mode’) which will make it difficult to fully comprehend information with consequent escalation of the threat response when patients do not receive what they are asking for. The ‘Minnesota arc’ model will be presented as a tool to use to have these difficult conversations.

Acute and Post-Acute Pain Management

Dr. Erin Krebs

This module will provide a practical approach to the patient with acute pain from the perspective of the primary care provider. The importance of individualizing the treatment and how to manage expectations especially when there is a mismatch between what the patient expects and what the provider can offer will be discussed. Other issues to be presented to include non-opioid alternative approaches to pain management and opioid tapering.

Chronic Pain Management

Dr. Charles Reznikoff

This module will focus on chronic pain management including the role of the pain management clinic and the treatment of opioid use disorder. Recommendations for when to refer a patient to a pain management clinic or an addiction specialist and what a referring provider and patient should expect will be discussed. Issues about what works well and what does not in the treatment of chronic pain will be reviewed. A practical approach to developing a team approach to treatment of chronic pain including non-opioid approaches will be discussed. A practical approach to using opioid prescribing guidelines will also be reviewed.

Module 4

Interventional and Non-Opioid Management of Pain

Dr. Clarence Shannon

This module will focus on the multidisciplinary approach to pain management including interventional and non-opioid management of pain. When and how to use devices such as pain pumps, nerve blocks and other interventions will be reviewed. Advice to providers who are referring patients to a pain clinic will be discussed including management of expectations of both the provider and patient. This discussion will include best practices and use of opioid prescribing guidelines.
